Information of Double Jaw Orthognathic Surgery in Madrid

  • All you need to know about Double Jaw Orthognathic Surgery in Madrid

Orthognathic surgery, from the Greek orthos meaning "straight" and gnathos meaning "jaw", is a procedure used to correct facial deformities caused by abnormal (excessive or insufficient) growth of the maxillary bones and jaw or chin.

the Dr. Jesús Manuel Muñoz Caro will be responsible for restoring harmony and proportion to the face—not only by correcting disproportions, but also by improving malocclusion and the function of the temporomandibular joint (TMJ).

Unlike single jaw surgery, in which only one bone is repositioned, double jaw surgeryinvolves modifying both the upper jaw (maxilla) and the lower jaw (mandible). Access to these areas is achieved through intraoral incisions, which means there will be no visible scarring on the face.

The operation lasts between 2 and 4 hours, is performed under general anaesthetic, and is suitable for patients aged 16–19 and above. However, it is generally recommended to wait until full maxillofacial development is reached—usually between the ages of 20 and 21.

  • This procedure is performed on patients whose facial bone growth — particularly the upper maxilla and the mandible or chin — has not developed correctly.

Double jaw orthognathic surgery is indicated in patients with functional problems involving the mouth, or those who present with facial asymmetry.

The facial deformities that justify the need for double jaw orthognathic surgery include:

    • Retrognathia or class II malocclusion: A developmental alteration of the mandible in which it is positioned further back than the upper maxilla.
    • Mandibular prognathism or class III bite: A condition where the mandible is positioned forward in relation to the upper maxilla.
    • Open bite: The upper and lower teeth do not overlap. Typically, only the molars come into contact.
    • Facial asymmetry: A skeletal imbalance caused by either excessive or insufficient growth of the maxilla or the mandible.
    • Severe gummy smile: Characterised by 6 millimetres or more of visible gum tissue and a vertical overgrowth of the upper maxilla, which often results in a disproportionately long and less harmonious facial appearance.

In addition to the aesthetic concerns these anomalies present, they also lead to a number of issues that affect the patient's quality of life:

    • Speech difficulties
    • Problems with eating and chewing
    • Breathing difficulties
    • Temporomandibular joint (TMJ) disorders

If you're considering surgery as a solution to your maxillofacial problem, you likely already have a diagnosis.

However, Operarme offers a first surgical assessment consultation with the Dr. Jesús Manuel Muñoz Caro to determine whether you are a suitable candidate for the surgery.

    Make appointment
  • During this initial surgical assessment consultation, the specialist will examine you and confirm whether double jaw orthognathic surgery is the most appropriate treatment for your case.

This consultation, offered by Operarme, takes place at the Hospital Viamed Santa Elena in Madrid.

Although you will likely already have a diagnosis and relevant medical tests, the specialist may recommend further imaging to better assess your jaw condition.

These additional tests (orthopantomography, 3D imaging, and maxillary CT scan) are not included in the package price. Therefore, it is essential that you bring all relevant medical tests and your clinical history with you. This will allow for a more accurate surgical diagnosis and avoid any extra costs.

During the consultation, the specialist will answer any questions you may have about the procedure and explain which type of treatment best suits your needs—whether it's the conventional approach (with orthodontics before and after surgery), or the surgery first method (where orthodontics is only needed after surgery).

If, by the end of the consultation, you decide to move forward with the maxillofacial surgery, the Dr. Jesús Manuel Muñoz Caro will propose a date for your pre-operative tests, which are included in the price of the surgery.

Pre-operative assessment at the Hospital Viamed Santa Elena

The pre-operative assessment for double jaw orthognathic surgery will take place at the Hospital Viamed Santa Elena in Madrid. This stage is included in the total cost of the surgery and consists of the following tests:

    • Electrocardiogram: to assess your cardiovascular health.
    • Full blood test: to check for any infections that may need to be treated prior to surgery.
    • Chest X-ray: to rule out signs of infection and confirm the correct positioning of the trachea. This test will only be performed if the doctor deems it necessary.

Once this stage is complete, you will need to attend a consultation with the anaesthetist at the Hospital Viamed Santa Elena.

Since double jaw orthognathic surgery lasts between 2 and 4 hours, it must be performed under general anaesthesia.

During this consultation, the anaesthetist will review your test results and ask you questions about lifestyle factors that may affect the procedure. These questions will cover topics such as your diet, any regular medications, and your alcohol or tobacco use.

  • Double jaw orthognathic surgery is performed by a specialist in maxillofacial surgery at the Hospital Viamed Santa Elena in Madrid.

the Dr. Jesús Manuel Muñoz Caro will carry out the maxillofacial procedure as follows:

    1. Once the anaesthesia has taken full effect, the surgeon will access the upper jaw through an incision at the front of the mouth.
    2. He will then cut and reposition the upper jaw until it reaches the desired position.
    3. It will be fixed in place using titanium mini plates and screws.
    4. In the same way, the specialist will access the lower jaw, this time through an incision behind the molar.
    5. A bilateral osteotomy of the lower jaw will be performed using a small surgical saw.
    6. This will then be fixed in place using mini plates and screws.
    7. Finally, all incisions will be closed with sutures.

All incisions made during double jaw orthognathic surgery are intraoral, which means there will be no visible scarring on the face.

Following the maxillofacial operation, you will remain at the Hospital Viamed Santa Elena for one night.

24 hours after surgery, if everything is progressing well, the medical team will discharge you so you can continue your recovery from the comfort of your home.

Additionally, Operarme offers a free transport service to take you home, so you can focus solely on your recovery without any added stress.

  • The results of maxillofacial surgery will gradually become visible. It is essential that you follow the medical instructions provided, as they will support your recovery process:

    • Follow a liquid diet
    • Wear elastic bands fixed to the jawbones
    • Sleep in an upright position – it’s common to experience some difficulty breathing after surgery due to swelling

After two weeks, the most noticeable discomfort following the double jaw procedure should subside.

You’ll be able to move on to a soft diet, and by the end of the second month, all symptoms are expected to have disappeared completely.

Benefits of double jaw orthognathic surgery

The benefits associated with double jaw orthognathic surgery include:

    • The facial deformity is corrected, resulting in more symmetrical facial features and improved aesthetics and function in the mouth area.
    • Functional improvement. After maxillofacial surgery, the bite is corrected, reducing the likelihood of future problems.
    • Aesthetic improvement. Jaw and maxillary deformities are eliminated.
    • Minimally invasive technique. The incisions are made intraorally, which reduces postoperative swelling and pain. Furthermore, there are no visible scars.

The postoperative risks associated with double jaw surgery are similar to those of any surgical procedure, such as infection, complications with anaesthesia, or the possibility of bleeding.

However, the likelihood of these complications is significantly reduced when the procedure is performed by an experienced maxillofacial surgeon.

  • The cost of Double Jaw Orthognathic surgery is 17.990 € and includes everything necessary to carry out the procedure with the highest level of safety and the best possible outcome.

The items covered in the cost are designed not only to ensure medical security but also to provide comfort throughout the process.

Therefore, the price of orthognathic surgery with Operarme includes the following:

    • Initial surgical assessment consultation
    • Reservation and use of the operating theatre
    • Complete preoperative tests
    • Surgeon's fees
    • Nursing team fees
    • All surgical materials required for the procedure
    • Anaesthesia and access to the recovery room
    • Two postoperative check-ups
    • Free transfer service
    • Medical Complications insurance
    • Full process management and personalised care
