  • As we have been saying, one of the main problems caused by varicocele is male infertility, among other complications, which is why it is necessary to treat it.

    To solve the problems caused by varicocele, specialists can resort to different treatments, but the definitive remedy is surgery. As far as surgical treatment is concerned, the operation is adapted to the degree of varicocele and the characteristics of the patient.

    Inguinal varicocele surgery is the most common technique for solving testicular varicocele problems.

    Mainly, we can differentiate between three types of varicocele operations, these are:

    • Open varicocelectomy. This is usually performed on an outpatient basis, under general or local anaesthesia. The surgeon will reach the vein through the groyne (inguinal or subinguinal), although he can also make an incision in the abdomen or below the groyne. He may use a surgical microscope to help him see the area better, or he may use Doppler ultrasound to guide the procedure. Once in the vein, he will ligate the dilated veins of the internal inguinal ring. This is the most commonly performed treatment for problems caused by varicocele. The inguinal varicocele operation aims to separate the artery and lymphatic vessels to prevent hydrocele formation.
    • Laparoscopic varicocelectomy. This is a minimally invasive technique that makes it possible to observe the inside of the area and identify the affected structures, dissecting the inflamed vein without intervening in adjacent structures.
    • Percutaneous embolisation. This is a procedure performed by the radiologist. A tube is inserted into a vein in the groyne or neck, called a catheter, through which instruments can pass. Using x-rays for guidance, the doctor threads the catheter into the varicocele and places a coil that blocks blood flow to the affected testicular vein and directs it to normal veins, repairing the varicocele.

    It is important that on the day of the consultation you bring all the medical tests you have based on the problems caused by the varicocele to allow the specialist to make a better diagnosis.

  • On the day of the pre-operative varicocele operation, you will hand in your paperwork at the hospital reception desk and then you will be accompanied to undergo the necessary pre-operative tests:

    • Complete blood test: To determine if there is any infection and if there is any associated problem that could compromise the operation.
    • Electrocardiogram: To determine the health of the patient's heart.
    • Chest x-ray: To determine if there is any pulmonary pathology and if there is any problem with the patient's rib cage.

    After the preoperative, normally on the same day, a consultation with the anaesthesia specialist will be carried out to determine and explain the type of anaesthesia to be used during the varicocelectomy, which is regional in most cases, although in some cases it may be general, as well as to find out more about your lifestyle (if you are a smoker, alcohol intake, diet, etc.) in order to carry out the operation as safely as possible.

  • On the day set for the varicocele surgery at the La Salud Hospital, you must first take the necessary documentation to the admission area. The hospital staff will then provide you with a single room with an accompanying bed for admission.

    After the preliminary preparations, you will be provided with operating theatre clothes and you will be taken to the operating theatre for the varicocele operation. We will explain it to you step by step:

    Varicocele surgery is performed under regional anaesthesia.

    1. First of all, in the operating theatre, you will be given anaesthesia and once you are under its effects, the operation will begin.
    2. Then, if necessary, the inguinal area will be shaved and the incision to access the inguinal canal will be made.
    3. The surgeon will make an incision of about 5 cm and then extract the spermatic cord with a special surgical instrument, open it and ligate the internal veins that cause part of the problem, preserving the artery, the lymphatic vessels and the vas deferens. The external spermatic veins are then ligated.
    4. Finally, the surgeon reintroduces the venous plexus into the inguinal canal, sutures and repairs the incision site and completes the varicocelectomy.

    The duration of a varicocelectomy is approximately 20 to 60 minutes, depending on whether it is a unilateral or bilateral varicocele.

    After waking up, you will be taken to a recovery room where you will remain until you are fully recovered from the anaesthesia and your vital signs are normal.

    Although this is an outpatient operation, it is advisable that you remain hospitalised for at least 1 day after the varicocelectomy, to rest properly and to ensure that everything has gone well.

  • After the varicocele operation, the anaesthetist will wake you up and you will be transferred to the recovery room where you will stay for 1 to 2 hours. In this room you will finish recovering from the effects of the anaesthesia until your vital signs have stabilised.

    Once your vital signs have recovered, you will be taken to your room where you will be admitted for 1 day, according to medical indications. 

    During this type of recovery at the La Salud Hospital, the nursing team will explain to you the care you need to take to prevent the wound from becoming infected or the stitches from opening. Basically, you will have to avoid straining and clean the wound every day.

    The first consultation after varicocelectomy with the specialist will be after 10-15 days.

    If everything continues to go well in the hours following the operation and admission, the doctor will discharge you so that you can return home to continue recovering.

    After the varicocele surgery, you will need to rest for 1 - 2 days before returning to your daily activities, as long as these do not involve heavy physical exertion. Full recovery will take just 2-3 weeks.
