02. How is Thulium Laser Prostatectomy performed?
In order to address an enlarged prostate using the Thulium Laser, the surgeon has different techniques to choose from; however, all of them are performed via transurethral, using an adapted cystoscope under general anaesthesia, and previously introducing a serous fluid that allows expanding the walls of both the bladder and prostate urethra, so the surgeon can get a better view and be able to manoeuvre more easily the laser:
Vaporisation of the prostate
This technique evaporates the enlarged prostate tissue by heating it. This phenomenon occurs when the intracellular water in the tissue of the prostate is heated by the Thulium laser, until it rises to 100 degrees. During this process, the heated water inside the cell evaporates along with the rest of the cellular material.

Vapo-Resection of the prostate
This technique combines the aforementioned vaporisation of the tissue with tissue resection using a laser beam.
This technique is highly effective in medium to large prostates since it allows vaporisation while creating tissue fragments as in transurethral resection treatment.
Thulium laser prostate surgery is the most advanced technique in the treatment of BPH nowadays.
In both procedures, a bladder catheter is placed in the patient’s bladder after the intervention to remove the fluid and any debris that may remain after vaporisation and resection of the prostate.
The duration of the surgery is less than in conventional prostate surgeries, and it usually takes around 30-45 minutes.
03. Post-operative of Thulium Laser Prostatectomy
Once the procedure has been completed, the patient is taken to a recovery room where remains until his/her vitals have stabilised.
The patient remains at least 24 hours in the hospital with a catheter on to help you urinate.
You will be discharged from hospital when you regain normal urination.
In this case, bleeding will be minimal, thanks to thulium laser technique that prevents it, which is why the hospital is short.
Full recovery time for this intervention is around 3-4 days after surgery.

The main advantage of prostate surgery is the solution of Benign Hyperplasia Prostate symptoms: these could get worse by time if there’s no surgical treatment.
More advantages of undergoing surgery with thulium laser technique to reduce the size of the prostate are the following:
- Faster laser: the laser performs the function of vaporisation and resection at a higher speed than its predecessors, such as the green laser.
- Less bleeding: when using this laser, the heating disperses over the non-vaporised tissue producing an haemostatic effect (like cauterising a wound); therefore, avoiding the bleeding that causes prolonged hospital stay, as happens in conventional prostate interventions.
- It allows the surgeon to know the amount of operating tissue.
- The duration of this intervention is shorter than in other prostate procedures.
- The hospital stay is short. Discharge happens after 24 hours of having the surgery done, due to minimal bleeding.
- This technique also reduces erectile dysfunction and urine incontinence.
- Medical complications during and after surgery are less likely. While complications in more conventional surgical techniques are quite unlikely and even rare, with Thulium laser, the chance of experiencing any surgery-related problems is further reduced.
- Thulium laser prostate surgery also reduces complications for patients at risk: patients with heart diseases, of older age, or who are taking anticoagulants can undergo this intervention without added risks.
