The frenulum is a layer of skin that joins the glans penis to the inner part of the foreskin in order to limit the sliding of the foreskin over the glans penis. However, when the problem of a short frenulum occurs, the correct sliding of the foreskin over the glans is altered, causing pain during erection, bleeding and even the possibility of the foreskin breaking during sexual intercourse.
It is possible to solve the anomaly with exercises and ointment applications that help to improve the skin's elasticity, although the most effective solution is surgery. Depending on the type of problem, circumcision, frenuloplasty or frenulectomy may be performed.

02. What is short frenulum?
A short frenulum is an abnormality affecting the length of this structure of the penis that joins the foreskin to the glans penis. When this is the case, it is perceived as difficulty in pulling the foreskin back, making it impossible to uncover the glans completely.
It is possible to develop short frenulum preputialis due to a number of factors, the most common being:
- Rupture of the frenulum: there is the probability that the rupture itself is caused by a short frenulum, but it is also possible due to an injury during sexual intercourse or masturbation.
- Infections: having suffered an infectious pathology in the genital area can lead to the development of inflammation and alterations in the frenulum tissue. A urogenital infection can cause the frenulum to shorten and the skin to thicken, preventing the foreskin from retracting properly.
- Phimosis: this abnormal growth of the preputial skin often means that the frenulum also develops incorrectly, becoming shorter than normal and leading to the need for circumcision.
- Genetics: it is a condition inherited congenitally from your parents, i.e. you are born with a shortened frenulum.
A short frenulum puts stress on the penis during erection, which is very uncomfortable and, in some cases, causes the glans to curve downwards.

Symptoms of short frenulum
- Pain. Caused by the tension exerted by the frenulum during erection or retraction of the foreskin. In cases of a very short frenulum, the pain can lead to impotence.
- Cracks in the skin of the foreskin. The skin has an abnormal appearance due to forced and constant tension of the skin. Tissue irritation and itching may also occur.
- Bleeding. It is normal when the frenulum breaks, leading to abundant and prolonged bleeding, requiring emergency medical attention for proper healing.
- Infection. This may be caused by an injury or incorrect hygiene due to difficulty in retracting the foreskin.
- Glans drooping during erection. Because of the tension exerted by the short frenulum, the foreskin cannot fully uncover the glans penis, wedging it between the tissue. This results in a curved appearance of the penis when erect.

05. Preliminary study of the short frenulum operation (frenuloplasty)
On the day set for the blood test, you will need to come to the hospital with the necessary documentation.

- Coagulation analysis: this allows us to determine whether or not the patient's blood is clotting correctly.
Normally, in more complex surgeries, several additional tests are carried out, such as a chest X-ray or an electrocardiogram. In the case of the short frenulum operation, as it is a very simple operation performed under local anaesthesia, it is not necessary.
06. Short frenulum operation (frenuloplasty): step by step
Frenulectomy is an operation that is performed under local anaesthesia and on an outpatient basis, so you can go home on your own after the operation. The short frenulum operation consists of the following:
When you enter the operating theatre, the team of doctors will instruct you to stand facing upwards, in the same way as women stand during childbirth, i.e. with your legs raised and spread.
You will be covered with a surgical drape, leaving only the penile area uncovered.
You will then be washed and disinfected and the local anaesthetic will be administered, waiting for it to take effect so that the frenulectomy can begin:
- Once the anaesthetic has taken effect, the specialist will section off the excess part of the foreskin, in cases where circumcision is required.
- The next step will be to cut the frenulum so that the area is relaxed and the necessary stitches will be placed. The stitches are absorbable and will fall out on their own.
- Finally, the area will be bandaged and the nursing team will explain the postoperative care of the surgical wound, as well as some medical instructions.
This is an outpatient surgery, so you will be able to go home when it is over.
The duration of this operation is approximately 20 - 30 minutes. As for recovery, it takes 2 - 4 weeks after the operation.

Do I have to take a PCR test before surgery?
