Information of Implantation of Artificial Urinary Sphincter in Madrid

  • Artificial Urinary Sphincter Implantation in Madrid

The implantation of the artificial urinary sphincter is a surgical procedure designed to treat urinary incontinence, especially in patients with urine loss exceeding 400–600 grams per day.

The artificial urinary sphincter is a three-component device: a pump, a cuff, and a reservoir. With the help of a manually controlled pump, the patient can regulate bladder function by compressing and decompressing a cuff placed around the urethra.

This surgery is performed at the Viamed Fuensanta Hospital in Madrid by the Dr. Manuel Fernández Arjona, a urology specialist with extensive experience in prostate-related conditions.

In this document, you will find all the relevant information about artificial urinary sphincter surgery in Madrid.

  • Male urinary incontinence (MUI) is defined as the accidental or involuntary loss of urine. It is not a disease in itself but rather a warning sign—an indication that there is a problem within the urinary tract.

The artificial urinary sphincter (AUS) is a silicone hydraulic prosthesis designed to restore continence without causing harm to the patient. However, the implantation of the artificial urinary sphincter is indicated only in cases of severe urinary incontinence, meaning when the amount of urine loss exceeds 400–600 grams per day.

Typically, patients who undergo this procedure have previously tried other surgeries or therapies that were unsuccessful. In fact, 90% of patients who receive an artificial urinary sphincter have previously been treated for prostate cancer.

The artificial urinary sphincter is a device that functions like a natural urinary sphincter and is composed of three components: an inflatable cuff placed around the urethra, a reservoir to hold the fluid, and a pump that controls the cuff. When the patient feels the need to urinate, they press the pump, which is located in the scrotum (for men) or in the labia (for women).

  • If you are considering the artificial urinary sphincter as a solution for your urinary incontinence, the first step is to schedule a surgical evaluation consultation with a Urology Specialist to determine whether this is the right treatment option for you.

Operarme offers you a surgical assessment consultation with the Dr. Manuel Fernández Arjona at the Viamed Fuensanta Hospital, so he can assess your condition and confirm whether the artificial urinary sphincter is the appropriate treatment for you.

In addition, if you have any questions about the surgery or the surgical process, you can contact the Operarme team through the contact form or by calling at +34 91 141 33 56.

After scheduling the consultation, one of our patient service advisors will contact you to answer any questions you may have and confirm that the date and time of the appointment suit your availability.

On the day of the consultation with the Dr. Manuel Fernández Arjona, you should bring all the medical and diagnostic tests you have related to your condition. The urologist will review your medical history, perform a physical examination (if necessary), and assess the medical tests provided (such as ultrasounds, blood tests, etc.).

After the consultation, our patient service team will contact you to ask how everything went and whether you have decided to proceed with the surgery. If so, they will explain the next steps, arrange your preoperative and anesthesia appointments, and assist you with any questions that may arise throughout the process.

  • The implantation of the artificial urinary sphincter is a procedure performed at the Viamed Fuensanta Hospital in Madrid by the Dr. Manuel Fernández Arjona

As mentioned earlier, in order to carry out the surgery, it is necessary to assess your overall health. Therefore, a preoperative evaluation will be conducted, which is included in the price of the surgery.

The preoperative tests for the artificial urinary sphincter include:

    • Complete blood work: This includes a full blood count, a basic coagulation study, and an assessment of kidney function. A urine test will also be performed to rule out infection.
    • Electrocardiogram (ECG): To evaluate your overall health status.
    • Chest X-ray: This test will only be performed if deemed necessary by the physician. It allows the doctor to check for any pulmonary conditions and to ensure that the trachea is properly aligned.
    • Consultation with the anesthesiologist: After completing the preoperative phase, you will have a consultation with the anesthesiologist to review the test results and discuss other important information such as allergies, medications, previous surgeries, etc. Based on this consultation, the anesthesiologist will determine the appropriate type of anesthesia for your procedure.

Once the preoperative phase is completed and the Dr. Manuel Fernández Arjona determines that there are no medical contraindications to performing the procedure, he will propose a date for the surgery—if one has not already been set—and all that remains is to attend the hospital for the implantation of the artificial sphincter.

The procedure to implant the artificial urinary sphincter is generally performed under general anesthesia so that you remain asleep and feel no discomfort during the operation.

Accordingly, the anesthesiologist will administer the necessary medication, and the placement of the device will involve the following steps:

    1. First, the doctor will insert a catheter to ensure that the bladder is empty during the surgery.
    2. Next, an incision will be made in the perineum and another in the suprapubic area.
    3. Then, the cuff will be placed around the urethra.
    4. After that, the reservoir will be inserted beneath the abdominal muscles.
    5. Finally, the pump will be placed in the scrotum (for men) or in the labia (for women) and connected to the other components of the device.

The medical team, which includes the Dr. Manuel Fernández Arjona, will leave the urethral cuff open and deactivated for several weeks to allow the tissues to heal properly.

The surgical procedure to implant the artificial urinary sphincter takes approximately 90 minutes.

After the surgery is completed, you will be taken to the recovery room to recover from the effects of the anesthesia.

Once the medication has completely left your system, you will be transferred to your hospital room, where you will remain for 24 hours to ensure that everything has gone well.
