How is a fusion-guided prostate biopsy performed?
1st Appointment
First, you will need to attend an assessment visit with the urology specialist who will carry out the procedure. During the consultation, the doctor will review your medical history and discuss your current state of health.
If the specialist determines that the test is necessary, you will need to undergo a Multiparametric Prostate MRI before the biopsy. Please note that this scan must be purchased separately. 2nd Appointment
Once you have purchased the multiparametric MRI, our team will assist you in scheduling the appointment for the scan. On the day of your appointment, the Magnetic Resonance Imaging (MRI) will be performed to identify suspicious areas for prostate cancer (CaP) or other abnormal cells.
You must bring these imaging results to the specialist on the day of the biopsy.3rd Appointment
As previously explained, during this appointment, the specialist will upload the MRI images into the ultrasound system, enabling the software to perform the image fusion. You will then be scheduled for preoperative tests and given the date for the biopsy procedure.
Fusion-guided biopsy allows for highly targeted sampling of only the suspicious areas, as well as access to difficult-to-reach regions not easily targeted by traditional techniques.
This fusion of MRI and ultrasound provides the urologist with a highly detailed 3D view of the prostate, enabling precise needle guidance to sample only the areas suspected of containing CaP.
4th Appointment
Once everything is ready for the procedure and you are in the operating room, you will be positioned on the examination table as instructed by the specialist. The anaesthetist will then administer a short-acting sedative.
- Once the sedation takes effect, an ultrasound probe will first be inserted into the rectum. Then, the urologist will insert the biopsy needle through the perineum.
- Guided in real time by the 3D ultrasound image (resulting from the fusion of MRI and ultrasound), the specialist will take microscopic tissue samples (thin cylindrical sections) from various areas of the prostate gland.
- During the procedure, the urologist will also perform a detailed mapping of the entire prostate to determine whether there is a single cancerous focus or multiple suspicious areas.
- After the tissue extraction is complete, the instruments will be removed and you will be taken to the recovery room.
The fusion biopsy takes approximately 30 minutes. You may return home around 2 hours later, once the effects of the anaesthetic have worn off.
The tissue samples are then sent to a pathologist to determine whether the biopsied suspicious areas contain cancer and, if so, to assess the degree of malignancy.
Results are usually available within 1 to 3 days. If cancer is detected, it will be evaluated to determine how quickly it may progress and what the most appropriate treatment options are for your case.

The primary aim of a fusion-guided biopsy is to detect prostate cancer (CaP).
Because prostate cancer often develops without symptoms in its early stages and has a high cure rate when detected early, it is essential to carry out diagnosis using a careful and efficient combination of all available tools.
Prostate cancer is the most common malignant tumour in men, but it also has a high survival rate when identified early.
The first step is the multiparametric MRI (mpMRI), which helps identify the area within the prostate that is most likely to contain cancer, using a scoring system called PI-RADS (ranging from 1 to 5).
Once suspicious areas have been identified, the specialist will assess whether a biopsy is necessary and, if so, determine the precise areas from which to take tissue samples.

When is a fusion-guided prostate biopsy recommended?
A fusion-guided prostate biopsy is recommended when initial tests—such as PSA (Prostate-Specific Antigen) blood levels and digital rectal examination (DRE)—are inconclusive or unclear. It is also a suitable option if a previous traditional transrectal biopsy has produced a false negative—that is, symptoms persist or worsen, and PSA levels continue to rise.
