Information of Robotic-Assisted Myomectomy (Da Vinci) in Valencia

  • Information about Robotic Myomectomy (da Vinci robot) in Valencia

Robotic myomectomy is the ideal surgical treatment to remove uterine fibroids using the minimally invasive technique offered by the Da Vinci Robot. This surgery is an alternative to hysterectomy, which involves the complete removal of the uterus.

Therefore, myomectomy—whether robotic, abdominal, or laparoscopic—is recommended for women who wish to become pregnant or, for other reasons, want to preserve the integrity of their uterus.

Currently, we perform this surgery at the La Salud Hospital by the Robotic Surgery Team using Da Vinci technology. This option allows the removal of large, numerous, and hard-to-reach fibroids.

This robotic technology was developed by NASA to perform interventions in hard-to-access locations, such as battlefields or the International Space Station. As you will read throughout the text, the robotic technique offers several key advantages, such as a short hospital stay and rapid recovery.

This procedure, like many others, can be performed with more conventional techniques. However, the Da Vinci Robot offers a series of benefits that you should not overlook.

If you have any questions regarding the management and advantages of Operarme for robotic myomectomy (Da Vinci Robot) at the La Salud Hospital, please contact us at +34 91 141 33 56.

  • During the consultation, the Robotic Surgery Team of Hospital La Salud will consider any factors, test results, or analyses you present to establish a more precise surgical diagnosis and determine the need for a preoperative stage.

That is why, as mentioned previously, you should bring your medical tests related to your case—such as an ultrasound and a medical report—so the specialists can assess the viability of the surgery.

If you do not bring these tests, since they are necessary to indicate whether surgery is appropriate, they may be performed at the clinic itself and will require payment. If you have any questions regarding this matter, please contact the patient service team.

To get to know and become familiar with your situation, questions about your lifestyle habits, disease symptoms, and related details will be asked. This information will also help determine if the Da Vinci Robot is the appropriate technology for performing the myomectomy.

    Scheduling the surgery date

It is likely that after the surgical evaluation consultation, you and the gynecology specialist will set the surgery date if you decide during the consultation to proceed with the robotic myomectomy

In any case, the patient service team at Operarme will contact you the day after the consultation to check how everything went and whether you have decided to continue with the process. If you do decide to proceed, they will explain the next steps, schedule your preoperative appointment, explain payment methods for the surgery, and more.

  • The set of preoperative tests included in the Operarme price for robotic myomectomy using the Da Vinci Robot consists of the following:

    1. Blood test: this includes a full blood count, coagulation panel, and biochemistry. Through this analysis, the anesthesiologist will assess your platelet levels, detect any coagulation issues and/or anemia, as well as identify potential infections or evaluate the concentration of electrolytes (such as sodium or potassium) in your blood.
    2. Electrocardiogram (ECG): this test provides the anesthesiologist with information about heart rhythm, conduction, structural abnormalities, and overall cardiac function in order to avoid any complications during surgery.
    3. Chest X-ray: this allows the anesthesiologist to check for any bone abnormalities in the chest area, ensure the trachea is properly aligned, and identify any signs of infection.

On the same day, or another day before the surgery, you will have a consultation with the anesthesiologist who will be present during the procedure.

During this consultation, they will review the preoperative test results to identify any factors that could impact the surgery and assess how to manage any potential risks.

Additionally, the anesthesiologist will create a medical history that includes your lifestyle habits, alcohol or tobacco use, and any regular medications you may be taking.

  • Once at the La Salud Hospital, the first step will be to submit the required documentation at the admissions desk. Don’t worry — our patient care team will have already informed you in advance about the documents you need to bring.

After providing everything needed, you will be taken to the operating area, where you will meet the Robotic Surgery Team of Hospital La Salud, the same team you met during your previous consultations before the robotic myomectomy.

The duration of the robotic myomectomy ranges from 30 to 120 minutes, depending on the specifics of each case.

The medical team performing the robotic myomectomy is led by the Robotic Surgery Team of Hospital La Salud. They operate the robot from the console, assisted by one team member who mobilizes the uterus and another assistant handling the Da Vinci Robot instruments.

Thus, the robotic myomectomy will be carried out as follows:

    1. The first to act is the anaesthetist you met during your consultation. They will administer the appropriate dose of medication, which will vary depending on your test results and your specific case of fibroids. As you know, the myomectomy is performed under general anaesthesia, regardless of the surgical technique used.
    2. Once the anaesthetic takes effect, the medical team will place the trocars for the robotic arms on your abdomen. These will be arranged in an arc from the right anterior superior iliac spine to the left, passing through the navel, with a port positioned for the assistant.
    3. When the fibroids are large, the tissue cannot be removed through the abdominal incisions. In such cases, they are extracted via the vagina, so the robot is positioned between your legs at a 45-degree angle from your position to allow better vaginal access.
    4. Next, the surgeon will view the pelvic structures and surrounding organs on the monitor to ensure everything is in order.
    5. The surgeon injects the fibroids with diluted vasopressin to reduce blood flow to the fibroid and minimise bleeding.
    6. An incision is then made in the myometrium, the middle layer of the uterine wall, using monopolar energy carefully without excess.
    7. The fibroids are removed using a traction and contraction technique until complete excision. Usually, the procedure starts with the fibroid closest to the cervix and gradually moves towards the uterine fundus.
    8. Typically, fibroids are removed through the incisions made for the Da Vinci instruments. However, when fibroids are disproportionately large, the best option is vaginal extraction. In these cases, a small opening is made at the top of the vagina, and the fibroids are removed through it.
    9. If several fibroids are removed, a drain may be left in place for one day, depending on your condition and the medical team's judgement.
    10. Finally, the robot is detached, and all instruments are removed so you can be awakened and taken to the recovery room to complete your recovery from the anaesthetic.

Once the robotic myomectomy is completed, the Robotic Surgery Team of Hospital La Salud will take you to the recovery room, where you will remain until the effects of the anesthesia wear off.

  • Recovery following robotic myomectomy, just like the duration of the procedure itself, will depend on the specific characteristics and complexity of your fibroids.

Below is a list detailing the different recovery times after robotic myomectomy at the La Salud Hospital:

    1. Hospital stay. As mentioned, this will be determined by the Robotic Surgery Team of Hospital La Salud based on your condition and how complex your surgery was.
    2. Hydration through IV. Immediately after surgery, the Robotic Surgery Team of Hospital La Salud will keep you hydrated via IV fluids. Six hours later, you’ll be started on a liquid diet.
    3. Semi-liquid diet. After confirming that you tolerate the liquid diet, you’ll begin a semi-liquid diet approximately 12 hours post-surgery.
    4. Recovery of digestive function. While on a semi-liquid diet, more solid foods will be gradually introduced until, around 24 hours after surgery, your digestive function is fully restored.
    5. Return to normal activities. As you've read, the number and size of fibroids will determine many postoperative factors, including your return to normal life. However, the recovery time after Da Vinci robotic surgery is significantly reduced—typically between 1 to 2 weeks.

The Da Vinci Robot represents advanced technology in the medical field, particularly in procedures like myomectomy, and offers numerous potential benefits. However, all decisions will be made based on your doctor’s clinical judgment.

Additionally, after you're discharged from the hospital, it’s likely that the Robotic Surgery Team of Hospital La Salud will provide you with specific medical guidelines, which we highly recommend you follow to support your recovery. Some of these may include:

    • Getting adequate rest and sleep
    • Taking short, frequent walks and gradually increasing distance
    • Avoiding lifting heavy objects or engaging in abdominal strain

Avoiding inserting anything into the vagina for the first 6 weeks. As mentioned before, your doctor will be the one to advise you on these matters, which may include avoiding vaginal douches, sexual intercourse, and the use of tampons.
