    Eye strain consists of the patient's loss of ability to focus on objects at different distances, although it is more common at short distances. This decrease in focusing ability is mainly due to the loss of flexibility of the patient's lens as well as the muscles that assist in the process.

    Presbyopia surgery with a trifocal lens consists of replacing the natural lens with an artificial intraocular lens that will perform its function, allowing the patient to regain the full range of vision.

    The surgical technique used is the same as the one employed today for any type of intervention involving the replacement of the lens with an intraocular lens: phacoemulsification. 

    This is a minimally invasive surgery that allows the entire process to be carried out through a small incision in the eyeball, at the edge of the cornea.

    Before the procedure, the surgeon will need to conduct a series of tests to determine the extent of the eye deterioration and decide what the best option is for the patient. They will also need to determine if it is indeed presbyopia, as it can be confused with myopia or hyperopia.

    Before the Procedure

    The patient will attend one or more prior consultations with the ophthalmologist to determine the exact conditions of the patient's eye, with the aim of performing the intervention with maximum safety and precision. Usually, the tests that will be carried out during the preoperative phase are:

    • In-depth eye examination
    • Pupilometry and measurement of intraocular pressure
    • Refraction with and without dilation
    • Orbscan topography
    • IOL Master biometrics
    • Specular microscopy

    A questionnaire will also be conducted to gather personal details that may interfere with the surgery. Among the questions asked are whether any previous eye surgeries have been performed or if the patient takes any medications or dietary supplements, whether prescribed or not.

    Additionally, the patient's lifestyle will be discussed to determine the suitability of monofocal, bifocal, or trifocal lenses for their way of life, although in the latter case, trifocal lenses are suitable for anyone as they allow for the full range of vision to be restored.

    Finally, the surgeon will prescribe antibiotic drops to be used in the days leading up to the procedure to prevent infections.

     

    As mentioned earlier, the surgical technique used for presbyopia surgery with a trifocal lens is phacoemulsification. This technique is the same one used in all cataract surgeries where the natural lens is replaced with an intraocular lens.

    This procedure does not require hospitalization and is performed under local or topical anesthesia. The difference between the two is that local anesthesia is administered through a series of injections around the eye, while topical anesthesia consists of eye drops that numb the eye directly.

    • The patient enters the operating room and is placed on the surgical table. A surgical drape or gauze will cover the patient’s face, leaving only the eye to be treated. Anesthesia will then be administered.
    • Once the anesthesia takes effect, the surgeon will keep the patient’s eye open using a surgical instrument that holds the eyelids in place.
    • The ophthalmologist will then wash the eye and surrounding area with saline. During this process, the patient may feel pressure in the eye or even lose part of their vision temporarily, but there is no need to worry, as it is not a serious issue.
    • The surgeon will make a small incision of about 2 cm at the edge of the cornea. Through this incision, a probe will be inserted, which uses ultrasound to dissolve the natural lens. The remaining liquid is then removed from the eye with a cannula.
