    Long sightedness (hypermetropia) is a vision condition that causes blurred near vision. It’s caused by a defect in the cornea or the lens, which makes images focus behind the retina, resulting in what we commonly refer to as blurred vision. This condition can be easily corrected with simple converging glasses.

    Despite the straightforward solution, many people prefer not to wear glasses in their everyday lives. For those individuals, laser eye surgery offers an effective alternative. Just like with short sightedness (myopia), the correction of long sightedness is done using LASIK laser technology—a minimally invasive, simple procedure that takes no more than 15 to 20 minutes.

    The development of long sightedness is unavoidable. It is a congenital condition that may stem from issues with the cornea, the lens, or the length of the eyeball. In younger people, symptoms often go unnoticed because the lens and surrounding muscles compensate by correctly focusing images on the retina. However, as we age, those muscles lose flexibility, and the ability to focus diminishes.

    At that point, there are only two options: wear glasses with converging lenses or undergo laser surgery (LASIK) to correct long sightedness.

    Once the anesthesia has been administered, the patient is advised to keep their eye closed to prevent dryness due to the temporary loss of sensitivity.

    Inside the operating room, it is very important for the patient to follow all the instructions given by the surgeon. The first step is to lie down on the operating table. The patient's face is then covered entirely, leaving only the eye to be operated on exposed.

    A surgical instrument is used to keep the eye open during the procedure by gently holding the eyelids in place.
    Next, the surgeon cleans the eye and surrounding area with a saline solution. The patient may feel some pressure on the eye or a temporary loss of vision during this step, but it is not serious.

    Using a device called a microkeratome, the ophthalmologist lifts the surface layer of the cornea. This layer is about one-tenth of a millimetre thick and is lifted to allow the laser to reshape the cornea underneath. The lifted flap remains attached to the eye by one of its edges throughout the procedure.

    The laser is then activated and applied to the corneal surface. This step usually takes less than a minute. The patient does not see the laser, only hears the characteristic sound of the machine. At this point, the surgeon will ask the patient to fix their gaze on a guiding light, even if their vision is blurry, as this is crucial for a successful outcome.

    Finally, the surgeon rinses the eye again and repositions the corneal flap. No stitches are needed because the flap naturally adheres back into place.

    Pre-operative Assessment for LASIK Surgery

    To ensure the surgery is performed with maximum safety and effectiveness, the ophthalmologist will perform a series of tests, including:

    • Fundus (retina) examination, pupil measurement, and intraocular pressure check
    • Prescription measurement with and without pupil dilation
    • Orbscan topography
    • Schirmer's test
    • Biometry

    During the assessment, the ophthalmologist will also ask if the patient has undergone any previous eye surgeries or if they are taking any medication that could affect the eyes.

    Patients must stop wearing contact lenses for at least two weeks before surgery. This period may vary depending on whether they wear soft or hard lenses.     There are certain cases in which LASIK surgery for long sightedness is not recommended. These include patients with serious eye conditions such as corneal irregularities (detectable through topography), cataracts, or uveitis. 

    Pregnant women are also not suitable candidates, as hormonal changes during pregnancy can affect vision prescription. In addition, LASIK for long sightedness is only suitable for patients with a prescription between +1 and +5 diopters.

    Step-by-step: LASIK surgery for long sightedness

    The LASIK procedure is performed on an outpatient basis using topical anesthesia, administered in the form of eye drops.

    1. Once the anesthesia has been applied, the patient is instructed to keep the eye closed to prevent dryness due to reduced sensitivity. 
    2. In the operating room, the patient must follow all instructions given by the surgeon. They will be placed on the surgical table, and the eye that is not being operated on will be covered with a sterile drape or gauze.
    3. A surgical instrument will be used to gently hold the eyelids open throughout the procedure to prevent the eye from closing.
    4. Next, the surgeon will rinse the eye and surrounding area with saline. The patient may feel pressure in the eye or even temporary loss of vision—this is completely normal and not a cause for concern.
    5. The ophthalmologist then uses a device called a microkeratome to lift the superficial layer of the cornea, which is about a tenth of a millimeter thick. This allows the laser to be applied directly to the corneal bed. The flap remains attached to the eye by one of its edges.
    6. The surgeon activates the laser, applying it to the exposed cornea for less than a minute. The patient does not see the laser but may hear the characteristic sound of the machine. At this stage, it is crucial for the patient to focus on a guiding light, even if their vision is blurry, to ensure the procedure is carried out correctly.
    7. Finally, the surgeon rinses the eye again and repositions the corneal flap. No stitches are needed, as the flap naturally adheres back into place.

    The entire LASIK surgery for long sightedness takes approximately 15 to 20 minutes from the time the patient enters the operating room to when they leave. The procedure is completely painless both during and after the surgery.
