01. What is a spermiogram?
A spermiogram, also known as a seminogram, is a fertility test that helps determine whether or not male infertility is present in a patient.
It provides crucial information about possible infertility in a couple, as it can indicate the quantity, quality and motility of the spermatozoa present in the patient's semen.
It is a simple and painless test that requires guidelines and indications for the test to be carried out and for qualitative and reliable results to be obtained.
02. When is a spermiogram necessary?
The spermiogram or seminogram test is performed in various situations to clarify the degree of male fertility in a man, from fertility treatments for couples to have children, to follow-up tests after surgical interventions such as vasectomy.
The most common situations are:
- After vasectomy surgery: in order to monitor and ensure the success of the surgery, the patient undergoes a seminogram test to check for the absence of spermatozoa in the semen.
- To determine if there are fertility problems: one of the most common situations in which this test is performed is when a couple is having difficulty conceiving. For this reason, this male fertility test is performed to determine the viability and quality of the sperm in the male.
- Fertility study: when a couple decides to undergo fertility treatment in order to conceive a child, they must first undergo preliminary tests, including a spermiogram. In this situation, it is very important to determine the quality of the man's semen and the quantity of spermatozoa.
03. How is a spermiogram performed?
The procedure is simple and generally only requires the following instructions for obtaining a semen sample:
- The patient must abstain from ejaculation for 3 to 5 days prior to sample collection to ensure a representative sample and to obtain an accurate analysis.
- The method of obtaining the sample can only be through masturbation; other methods such as coitus interruptus will not be valid.
- It is important to urinate and wash hands and genitalia before proceeding with the collection of the sample to ensure that the sample is not contaminated. No ointment should be applied.
- The patient should place the specimen obtained in a sterile container (these "sterile wide-mouth bottles" can be found in any pharmacy). Samples obtained in condoms will not be accepted.
- Once the sample has been obtained, the patient must make an appointment first with the Laboratory selected and take the sample. No more than 60 minutes should elapse from the time the sample is obtained until it is delivered to the laboratory. It must be ensured that the glass containing the sample is tightly closed and that the sample does not lose body temperature (between 36º and 37º). It is recommended to keep the bottle in a pocket close to the body.
It is very important to bear in mind that the sample must be complete, i.e. half or part of it cannot be spilled, as in this case, the laboratory would not consider it valid for analysis.
On the other hand, if the patient has suffered from fever in the days prior to the test, he/she should wait as this factor could alter the results obtained from the sample.
In the case of patients who have undergone vasectomy surgery and need to perform this test as a follow-up of their intervention, they should also take into account the following indications:
- Before collecting the sample to be analysed, you must remain sexually abstinent for a minimum period of 48 hours.
- You should bear in mind that 3-4 months must have elapsed after the vasectomy operation and before the first analysis, and you must have had a minimum of 25 ejaculations.
- The first analysis will be done 3-4 months after the operation and the second, two to four weeks after this first sample.
04. Possible spermiogram results
The spermiogram analysis studies different parameters that help the specialist to determine the best treatment for the patient (in the case of undergoing infertility treatment, for example):
- Sperm mobility: One of the most relevant aspects analysed, as sperm mobility is essential to be able to reach the egg and thus facilitate its fertilisation. The higher the proportion of motile spermatozoa, the greater the chances of fertilising the egg.
- Semen volume: the normal volume of a semen sample is between 2 and 6 millilitres. A volume lower than these parameters could indicate possible problems in the glands that produce the semen.
- Sperm morphology: Spermatozoa should have a normal appearance in structure and shape, i.e. they should have a head, neck and tail. In the case of an abnormal appearance, it can affect the fertility of the egg, especially if the percentage is significant.
