Prenatal diagnosis allows early detection of congenital defects in the foetus. At present, a combined first trimester screening programme for autosomal trisomies is offered to pregnant women:

• Ultrasound measure of the nuchal translucency.
• Determination of the levels of two hormones in maternal blood.
• Assessment of other data such as the mother’s age.

When this analysis concludes in a high risk of chromosome alterations, it recommends the performance of a non-invasive prenatal test, amniocentesis, or a chorionic villus sampling.

Harmony Prenatal test is a screening non-invasive test advisable in expectant mothers who present a high risk of aneuploidies, considering this situation a risk when the following requirements:

• Advanced maternal age (over 30 years).
• Ultrasound suspicion of aneuploidy.
• Previous history of pregnancy with trisomy.
• Positive results of first or second trimester prenatal screening.
• Existence of balanced Robertsonian translocations in the parents involving chromosomes 13 or 21.

The Harmony NIPT (basic) is indicated for expecting women under at least 10 weeks gestation, that present a single pregnancy or twin pregnancy, including pregnancies through egg donation.

In the case of twin pregnancy, only the chromosomes 21, 13 and 18 will be analysed; sexual aneuploidies won’t be detected. Also, the foetal sex shall be reported, provided that one of the babies is a boy, or both babies are girls.

Step by step of the testing

1. Before having this test done, it is advisable to have an ultrasound in order to know the exact date of the pregnancy, and therefore avoid possible anomalies that modify the parameters of the test and resulting in wrong data.
2. Once the ultrasound is performed, the non-invasive prenatal test is performed, consisting in a blood analysis. 

This is sufficient to obtain the results after testing the sample. This type of process reduces the number of subsequent follow-up tests resulting from a positive result.

After the blood collection, the sample will be sent to the Laboratory for its analysis, and you will need to wait a few days for the results.

Once the sample is collected and sent to the Laboratory for its analysis, the group of experts from Megalab Laboratories will prepare a results report that will be available in 5-8 working days.

• You will be provided with a password to access your private area.
• From there, you will be able to check the status of the results report.
• In case they are not yet available, it will appear as "pending".

After the blood collection and its analysis, the results reflected in the test report may reveal the following information:

• Negative result: discards the possibility of Down Syndrome, trisomies 18 and 13, and other alterations in the sexual chromosomes with maximum reliability, which is 99,9%, not 100%, since non-invasive prenatal tests are not diagnostic tests. The reliability for Down's syndrome is superior to the triple prenatal screening currently performed in the public health system (sensitivity 85%) or to screening plus genetic ultrasound (sensitivity 90%).
• Positive result: you should then take invasive diagnostic tests such as amniocentesis or chorionic biopsy. The possibility of having a chromosomal alteration is very, very high, but not high enough to make decisions about the future of this pregnancy. Amniocentesis has a 0.5% risk of miscarriage, but positive test findings must always be confirmed.
• Test with no result: sometimes insufficient foetal cells are obtained in the maternal blood sample. At <5%. In this particular case, the test can be repeated. In half of them, we get a result on the second attempt (at no additional cost to you). Finally, in 0.1-2.5% of cases, we do not get a result (depending on the brand used).

However, in the case of Harmony testing the rate of inconclusive results in less than 1%.

If the results are positive and there’s risk for anomalies, you can have a consultation with the laboratory’s genetic counsellor, via call, so that he/she can inform you about the implications of the results for your baby.
