Everything you need to know about Pilonidal Cyst surgery in Vigo
A pilonidal cyst, or sacral cyst, is an abnormal cavity caused by the accumulation of skin and hair, located at the base of the spine, around the sacral bone. This condition is characterized by a chronic infection in the sacrococcygeal area, just above the intergluteal cleft.
A sacral fistula occurs more frequently in young white men with a significant amount of body hair.
Because sacral fistulas can become very painful and uncomfortable, the most important thing is to seek treatment as soon as possible, to prevent the cavity from becoming deeper and more complex. In some cases, symptoms can be resolved with a small incision and drainage of the cyst.
When the initial treatment is not effective, the recommended approach is complete surgical resection of the sacral fistula—pilonidal cystectomy—which is considered the definitive surgical procedure to eliminate the abscess.
In Vigo, the pilonidal cyst surgery is performed by the Dr. Juan José Guede Merino.
Below, we explain all the details about the surgical resection of the sacral cyst, the benefits of this treatment, and how you can manage the entire process through Operarme.
-
Why is surgical removal of a pilonidal sinus performed?
In most cases, surgery is recommended when incision and drainage do not yield satisfactory results and you continue to experience issues with the pilonidal cyst.
The most common cause of its appearance is the embedding of a hair, which lodges and infects the area until it forms a cyst.
Therefore, surgery is considered the most effective way to eliminate the pilonidal sinus. This procedure involves removing the entire cyst as a whole, leaving the wound open to heal from the inside out, completely closing the cavity.

Request a surgical assessment consultation in Vigo
Finding an effective remedy for pilonidal sinus can take time and may involve several steps along the way. If conservative treatment doesn’t work for you, then the best solution is to consult a specialist about the surgical options best suited to your case.

Preoperative process for Pilonidal Cystectomy at the Concheiro Centro Médico Quirúrgico
On the scheduled day for the preoperative assessment of your pilonidal cyst surgery, you will need to go to the Concheiro Centro Médico Quirúrgico.
After submitting the necessary paperwork, you will be guided through the three tests required to determine the type of anesthesia to be used and to confirm whether you are fit for surgery:
- Electrocardiogram (ECG): to assess the condition of your heart.
- Chest X-ray: not always required; it must be specifically ordered by the doctor. This test is used to check for signs of lung disease and to confirm proper alignment of the trachea.
- Complete blood test: includes a full blood count, a basic coagulation study, an assessment of kidney function, and a glucose test. A urine test will also be conducted to rule out any infection.
- Consultation with the anesthesiologist: During this appointment, inform the anesthesiologist about any allergies you may have, any medications you are currently taking (prescription or over-the-counter), previous surgeries, etc. Based on this consultation, the appropriate type of anesthesia for your surgery will be determined.
If the anesthesiologist gives clearance for surgery, the next step will be to attend the hospital for the actual procedure.
-
Pilonidal Cyst Surgery: Step by Step
The surgery for a sacral cyst is typically performed using the Pilonidal Cystectomy technique. This is the most common surgical procedure to definitively resolve issues caused by a sacrococcygeal fistula.
Upon arriving at the clinic, you will need to provide all the necessary administrative documents (the patient service team will assist you personally with this) and identify yourself as an Operarme patient.
Next, you will be escorted to your private room, which includes a companion bed, where you will remain until it's time for your procedure, or you will be taken to a waiting room until you are called into the operating room.
Once you are called, you will enter a room where you will change into surgical attire and have an intravenous line placed for administering medication and anesthesia. Afterward, you will be taken to the operating room.
Pilonidal cyst surgery is performed under general or regional anesthesia.
Once in the operating room, you will again meet the specialist, the anesthesiologist, and several members of the nursing team, who will be ready to begin the procedure:
- First, the anesthesiologist will administer general or regional anesthesia.
- Once the anesthesia takes effect, the surgeon will make an incision in the area to remove the skin with the pores and the underlying tissue containing the hair follicles.
- After removing the fistula, depending on how much tissue is excised, the area may be filled with gauze or left open. This will mark the completion of the surgical procedure.
In some cases, a tube is placed to drain the fluid that accumulates after surgery. The tube will be removed later when the fluid no longer drains.
The duration of this procedure is approximately 30-60 minutes and is typically performed under general or regional anesthesia.
Once the surgery is completed, you will be transferred to the recovery room, where you will stay until the effects of the anesthesia wear off. Afterward, you will be moved to your room, where you will remain under observation for the next 24 hours until you are discharged.
-
Postoperative care and recovery after Pilonidal Cyst surgery
As mentioned earlier, after the surgery and once you have recovered from the anesthesia, you will remain hospitalized for 1 day until medical discharge.
During your stay at the Concheiro Centro Médico Quirúrgico, the specialist and the nursing team will check that everything went well and will provide you with postoperative instructions.
One of the most important postoperative instructions is: keep the area clean and protected from friction or hair near the wound.
Some of the most common guidelines to ensure the success of the surgery and recover as quickly as possible are:
- Take pain medications and apply antiseptics to prevent infection.
- Keep the area around the wound clean. The bandages and gauze will be changed over the next few days at an outpatient center until the wound heals and closes.
- In some cases, it is recommended to shave the hair near the intervention area to prevent hair from entering the wound.
Full recovery after surgery typically takes 1-2 months, depending on the patient's healing capacity and how deep the cavity caused by the pilonidal cyst was.
Generally, your next follow-up visit with the specialist is after 15 days. The doctor will arrange this appointment, and our patient care team will assist you in managing it.

Advantages and Risks of Pilonidal Cyst Surgery
Pilonidal cystectomy, the surgery for removing a sacrococcygeal fistula, has as its main benefit the permanent elimination of a problem that causes significant discomfort to those affected.
Additionally, it is a relatively short procedure and, in many cases, is outpatient, meaning it does not require a hospital stay and generally involves a short recovery period.
The resection of the pilonidal cyst is generally safe. Therefore, the risk of complications is very low and occurs very rarely. However, it is important to be aware of and know what the risks are:
- Bleeding
- Infection
- Slow healing
- Recurrence of the pilonidal cyst (over 40% of cases may require reintervention)
In addition to the advantages of the surgical removal of the pilonidal sinus, there are also benefits to undergoing the procedure through Operarme, with the main advantages being:
- Free surgical consultation with a specialist at the Concheiro Centro Médico Quirúrgico
- Fixed price with everything included, guaranteed. From the very beginning, you know the final price
- Fully personalized care from our patient service department, guiding you and solving any doubts that may arise during the process, so that you never feel lost and can focus only on getting better
Pilonidal sinus surgery includes hospitalization and medical complication insurance, which covers complications, such as the need for an additional day of stay or reintervention.
For more information on this, contact our patient service team on +34 91 141 33 56.
