The placement of fixed vestibular orthodontics with metal brackets is a procedure that helps to re-establish the correct position of the teeth, which has developed abnormally, damaging both the functionality and the aesthetics of our smile.

During this treatment, any bite and dental alignment problems such as overcrowding, open bite, overbite or crossbite can be solved.

Below you can find more detailed information about this process, from the diagnosis to the results and benefits obtained after undergoing fixed vestibular orthodontics with metal brackets.

02. What is orthodontics with metal brackets?

A fixed orthodontic appliance is a dental correction system. This type of appliance is called fixed because the parts responsible for exerting force on the teeth, the brackets, are attached to the teeth and cannot be removed from the mouth without the expert hands of an orthodontist.

Orthodontic systems with metal brackets are usually fitted from the age of 10-12 years, when the teeth are permanent and dental positioning problems need to be corrected.

Orthodontics consists of several different parts that guarantee the correct functioning of the dental appliance. These elements are:

  • Brackets: small metal or ceramic pieces that are attached to the surface of the teeth and whose function is to reposition the tooth in the correct position.
  • Bands: these are adapted and cemented rings that are placed around the molars.
  • Tubes: these are similar to brackets that are placed over the molars and can be used as a substitute for bands.
  • Arches: these are wires of different thicknesses and materials that are placed over the brackets and their main function is to transmit force to the teeth.
  • Ligatures: these are fastening pieces that are placed over the archwires and allow them to be retained inside the slot of the brackets. These elements can be elastic in different colours or metal.

Thanks to fixed orthodontic appliances, the movement of the teeth is generated by means of the force exerted by the arch on the teeth through the brackets, moving the teeth in the bone until the desired position of all the teeth in both arches is reached.

03. Why is fixed orthodontics with metal brackets necessary?

Treatment with fixed vestibular orthodontics with metal brackets is necessary when there are problems related to the incorrect position of the teeth. Among the most common conditions that are solved with orthodontics, we highlight the following:

  • Dental malposition: this appears in the form of rotated teeth and crowding of the teeth. This situation occurs because the teeth are larger than the space they have to fit in the jaw or maxilla.
  • Diastema: this is the separation or spacing between teeth due to missing teeth or teeth that do not occupy all the space available to them.
  • Open bite: in those cases in which, when closing the mouth, the upper jaw does not make contact with the lower jaw.
  • Crossbite: occurs when the upper teeth do not fall slightly over the lower front teeth or outside the lower back teeth when biting, when closing the mouth.
  • Overbite: when closing the mouth, the teeth of the upper arch cover more than one third of those of the lower arch.
  • Underbite: when we close our mouth, the teeth of the lower arch protrude excessively or the upper arch is positioned too far back.
  • Shifted midline: this occurs when the imaginary line that divides the centre of the upper front teeth does not align with the teeth of the lower arch when the mouth is closed.

Before starting treatment, the specialist in orthodontics and dento maxillary orthopaedics will carry out various diagnostic tests to determine the specific condition of your teeth and will personalise the guidelines to be followed to solve your case.

04. What does orthodontic treatment with metal brackets consist of?

Orthodontics is a speciality of dentistry that aims to correct teeth and dental bones that develop in an incorrect position. Thanks to this treatment, problems such as the early loss of teeth due to poor hygiene and the appearance of caries or periodontal disease can be avoided, as well as the following pathologies:

  • Overstrained chewing muscles that can lead to headaches.
  • TMJ syndrome (temporomandibular joint disorder that causes pain and dysfunction of the joints and muscles of the jaw).
  • Neck, shoulder and back discomfort.

The treatment consists of a system of bands, brackets, wires and ligatures. This system is fixed around the teeth and acts by exerting pressure on all the teeth, gradually moving them into the correct position. Fixed vestibular orthodontics is adjusted monthly and usually lasts for an average of two years until the desired results are achieved.

05. Who is a candidate for fixed orthodontics with metal brackets?

As we mentioned at the beginning of this page, orthodontic treatment can generally be carried out from the age of 10 - 12 years, and is most indicated as soon as problems or anomalies in the development of the teeth are detected.

In the case of adolescents and adults, treatment with orthodontic appliances is also indicated, although the results may not be as good as in children, as the teeth are fully developed and it is much more complicated to readapt them to the correct position.

The orthodontist will determine the type of orthodontic treatment you need, taking into account a complete diagnostic study, examining your complete clinical and dental history, as well as carrying out a clinical examination, x-rays and photographs of your teeth and study models. Based on the tests, he will personalise and develop a treatment plan adapted to your needs.

06. Booking of a free assessment consultation

The first step you should take is to make an appointment for an assessment with our Dentistry Specialist through Operarme, this appointment is completely free, with no obligation and adapted to your availability. During the consultation you will be able to meet the General Dentist who will be the first to assess your case and who will be responsible for referring you, during the same consultation, to the Orthodontist who will diagnose and develop the treatment plan.

After requesting the assessment consultation through our website, our patient care advisors will contact you to confirm the date and time of the consultation, and will also be available to answer any questions you may have about the process. We remind you that you can request more information at any time by calling at +34 91 141 33 56 or using the contact form.

07. Consultation with the Orthodontic Specialist

On the day of the appointment and after the consultation with the General Dentist, you will be called in by the Orthodontist. He or she will carry out the relevant tests to help determine the condition of your teeth, identify the anomaly you have and rule out any complications or limitations that could affect the treatment. He or she will also inform you of the possibilities and results that you will obtain thanks to the placement of fixed vestibular orthodontics with metal brackets.

During this consultation, diagnostic tests such as a clinical examination, panoramic and lateral X-rays of the skull, photographs and study models will be carried out. We insist that you ask any questions you may have regarding orthodontic treatment during the consultation, this will allow you to make the best decision to solve the problem.

If you decide to start treatment with the specialist during the consultation, you will be able to set a date for the fitting of the appliance on the same day. In any case, our patient care team will contact you to see how the appointment went and to find out your decision. If so, they will inform you about the next steps to follow and will manage the whole process.

08. Diagnostic tests for fixed orthodontic treatment with metal brackets

Fixed vestibular orthodontic treatment with metal brackets, like all other orthodontic appliances, requires diagnostic tests to plan the procedure. In this way, treatment is fully personalised and adapted to the patient's needs. The tests that are carried out during this stage are:

  • Intraoral photographs: this consists of capturing only the mouth without showing the lips, making use of lip retractors. These images are used to record the start of the treatment, the intermediate steps and the final result.
  • Extra-oral photographs: this is a photograph that captures the patient's entire face. It is used to assess the patient's facial characteristics and features along with the patient's smile. This test allows us to evaluate the changes that will be achieved with the treatment.
  • Orthopantomography: this is a panoramic x-ray on a single film, which shows the jaws, jaw and teeth in a single general image of the mouth.
  • Lateral teleradiography: these are very sharp images of the patient's facial bones and teeth. They allow the angles and proportions of the facial bones to be measured and the relationship between the facial structures to be determined.
  • Study models: this is an exact replica of the shape and placement of each patient's upper and lower teeth. This test makes it possible to observe alterations in the size of the teeth, their shape, spaces between the teeth, as well as deviations or rotation of the teeth.
  • Articulator mounting: this is a device used to position the study models in the occlusal position (closed mouth) similar to that of the patient. It reproduces the maxillary-mandibular relationship.

09. Fixed orthodontics with metal brackets placement day: step by step

When the day comes for your braces to be fitted, you will meet again with the orthodontist who diagnosed your case. The specialist will have everything ready for the fitting of the orthodontic appliance. There is nothing to worry about, it is a fairly quick process, without anaesthesia and does not cause any discomfort.

The steps to be taken during the procedure are as follows:

  • Once in the dental chair, first of all, a dental prophylaxis is necessary. This involves cleaning and polishing the teeth to make it easier for the brackets to adhere to the teeth. Next, the dentist will place a dental separator together with a hoover and cotton wool to isolate the tooth surface and keep it as dry as possible.
  • The tooth surface is then prepared by acid etching. This creates a microporous area so that the bracket can adhere correctly. Once the ideal surface has been achieved, the excess acid is removed and cleaned.
  • The next step is to brush the adhesive onto the vestibular surface of the tooth.
  • Next, the composite is placed to fix the bracket and the bracket is positioned on the vestibular surface of the tooth. Next, any remaining composite is removed, leaving the surface polished.
  • Once the brackets are in place, the archwire and the ligatures are placed to hold and adjust it.

Once the brackets have been fitted, the odontologist will explain all the necessary care that you will need to follow over the next few days and will schedule the next visit.

This process usually lasts approximately 30 minutes per arch, that is, around an hour in the case of placing the system on both arches, always depending on the complexity and characteristics of each one.

10. Follow-up treatment of fixed orthodontics with metal brackets

After the orthodontic appliance has been fitted, you should visit your dentist every month to monitor your treatment. It is very important that you attend the scheduled check-ups, as during these appointments the doctor will be able to check that your progress is progressing correctly and, if necessary, adjust your orthodontics.

Bear in mind that if you do not attend your check-up appointments, the only thing you will achieve is to unnecessarily lengthen the treatment and even hinder the correct recovery of the position of your teeth.

We insist that follow-up appointments are scheduled in advance and are perfectly adapted to the patient's schedule and availability.

11. Care during fixed orthodontic treatment with metal brackets

It is very common for questions to arise during treatment, so we will try to answer the most common questions that arise at this stage:

  • How can you prevent mouth sores? Normally, due to the friction and inflammation after the placement of the appliance, small sores or wounds may appear. To avoid this, it is recommended to use special orthodontic wax. In addition, it is advisable not to eat particularly acidic foods such as citrus fruits, fried tomato, vinegar or spicy foods if these sores appear.
  • How to brush your teeth with braces? In addition to brushing your teeth after every meal, you should complete your hygiene routines with dental floss or interproximal brushes. This will effectively remove food debris and bacteria that often remain between the teeth after brushing.
  • What foods should you eat during the first few weeks? During the first few weeks it is common to feel discomfort due to the pressure exerted by the brackets on the teeth. So to avoid more significant pain, it is recommended that you eat foods that require less chewing effort, such as purées, soups, or foods with a very smooth texture, such as fish, tender meats or cooked vegetables.
  • What foods should you avoid during treatment? It is essential that you avoid sticky foods such as chewing gum or jelly beans, as they stick to the brackets and make oral hygiene difficult. Likewise, do not eat very hard foods by biting into them, as this will prevent the brackets from popping out and lengthening the process.

12. Completion of fixed orthodontic treatment with metal brackets

Once the desired results and dental position have been achieved, the orthodontic appliance will be removed, normally 18 months after fitting.

After the orthodontic appliance is removed, you will be fitted with a retention appliance. Don't panic, this is a very small piece, which cannot be seen and whose function is to reinforce the teeth so that they do not return to the position they were in. Specifically, it consists of a sheet of very fine wire cemented to the inside of the lower and upper incisors.

In addition, for the first few months you will have to wear removable retainers called transparent splints, which prevent tooth movement. After a period of time, determined by the specialist, this removable appliance should only be used at night.

13. Advantages and risks of fixed orthodontic treatment with metal brackets

Fixed vestibular orthodontic treatment with metal brackets stands out for several reasons, despite being the most traditional procedure. You should bear in mind that in addition to correcting alignment, levelling and occlusion problems, it is also a system that ensures effective, very precise and fast results.

Other advantages of this procedure with metal brackets are that it is the most economical system on the market, its material is the most resistant compared to other materials used for orthodontic appliances and it is effective more quickly even in the most complex cases of malposition.

As far as risks are concerned, they are not usually serious or long-lasting, the most common being difficulties in speaking or chewing during the adaptation period. Slight discomfort is also frequent as a result of the adjustment of the orthodontic system, which subsides after a few hours and with the help of painkillers prescribed by the dentist.

On the other hand, it is common for small sores or wounds to appear as a result of the appliance rubbing against the cheeks. As soon as your mouth adapts to the presence of the appliance, these discomforts will not appear again.
