In which cases is frenulectomy performed?

  • Frenulectomy is the surgical operation in which the frenulum brevis is removed, in order to loosen the area and eliminate any bothersome symptoms during erection or sexual activity.
  • The frenulum brevis is a pathology that appears in uncircumcised men. The frenulum and foreskin are joined together, so uncircumcised men with a short frenulum have difficulty retracting the foreskin completely.
  • Frenulectomy is a surgery that is performed under local anaesthesia and on an outpatient basis, so once the operation is over, you will leave the hospital.

Frenulectomy consists of cutting the frenulum of the penis in order to eliminate the tension caused by having a short frenulum, but what is a short frenulum? Well, it is the one that limits the movement of the foreskin over the glans penis, which can cause pain during erection or sexual activity. 

The frenulum is the triangle-shaped skin attached to the foreskin. Therefore, uncircumcised men with a short frenulum have difficulty retracting the foreskin completely, preventing the glans from being fully exposed.

Short frenulum most often affects uncircumcised men.

The tension caused by the short frenulum when the penis is erect is very uncomfortable and, in some cases, can cause the glans to deflect downwards resulting in a curvature of the penis.

As a result, the frenulum can be further shortened by the scar that forms after the wound heals. The risk of the same thing happening again and again is therefore very high. 

On the other hand, when sexual intercourse has not taken place, the signs of a short frenulum are manifested by a jumping foreskin when manually retracted, i.e. a foreskin that does not lag behind when retracted.

A short frenulum is often confused with phimosis, but it is the urologist who will determine the pathology on the basis of a physical examination. Once the problem has been diagnosed, treatments will be proposed to solve it. However, the purpose of each treatment is different, although circumcision usually includes frenulectomy.

However, this is not very effective and is not usually a recommended option. The frenulum, regardless of its size, cannot be stretched sufficiently, especially in cases where the frenulum is very short.

Therefore, the definitive option is frenulectomy. However, the procedure to repair the short frenulum comprises 3 surgical solutions: circumcision, frenulectomy and frenuloplasty.

  • Circumcision: this procedure consists of removing the foreskin of the penis that covers the glans penis in order to permanently expose it. Frenulectomy is usually also performed in this procedure.
  • Frenulectomy: this technique involves cutting the frenulum in order to free it. This operation has a purely functional purpose, i.e. the aim of frenulectomy is to remove the frenulum and loosen the area.
  • Frenuloplasty: this is the plastic surgery technique that repairs the frenulum, achieving a much more aesthetic result.

Frenulectomy step by step

Frenulectomy is a surgery that is performed under local anaesthesia and on an outpatient basis, so you can go home on your own after the procedure. However, we recommend that you come accompanied. 

The short frenulum surgery will consist of the following:

  • When you enter the operating theatre, the team of doctors will instruct you to stand facing upwards, in the same way that women stand during childbirth, i.e. with your legs raised and spread.
  • You will be covered with a surgical drape, leaving only the penis area uncovered.
  • You will then be washed and disinfected and the local anaesthetic will be administered, waiting for it to take effect so that the frenulectomy can begin.

Frenulectomy is performed under local anaesthetic so you will not feel any pain or discomfort.

  • Once the anaesthesia has taken effect, the specialist will section off the excess part of the foreskin, in cases where circumcision is required.
  • The next step will be to cut the frenulum so that the area is relaxed and the necessary stitches will be put in place. The stitches are absorbable and will fall out on their own.
  • Finally, the area will be bandaged and the nursing team will explain the postoperative care of the surgical wound, as well as some medical instructions.
